Nationally, admissions and crisis are rising for autistic people, and while we’ve improved discharges for those stuck in hospitals, preventing crisis remains a significant issue, especially for autistic people and young individuals.
Our Transforming Care Service partners with health and care organisations to prevent crisis and hospital admissions through better information, early identification tools, and community support. We strengthen the voice of patients, their families, and citizens to improve care (education) and treatment reviews (C(E)TRs) and care quality, and tackle health inequalities through the development of LEDER strategies and improvement programs such as AHCs, STOMP-STAMP, and epilepsy improvement.

We combine a flexible team of experts with a collaborative approach to support provider engagement and capacity planning, deliver discharge planning improvements, facilitate better training, develop autism strategies, deliver integrated care records, and enable improved data sharing. We provide a brokering service for specialist expertise and benefits analytics, and all our service products are underpinned by our proven change management skills.
Our team provides both breadth and depth of expertise, and we can access further skills and experience as required. We offer leadership support to help build an understanding of problems and co-produce strategies to solve them, benchmarking tools to facilitate comparative thinking, and advocacy support and signposting.
We can provide models for minimum standard training that cross organisational boundaries, senior convenor specialists, coordinate communities of practice, improve advocacy involvement, remove boundaries between health and social care, support the voluntary and community sector, and build connections with people with lived experience.
