Our Personalised Healthcare Commissioning team provides expert children and young people’s (CYP) services to five ICSs and is one of the largest national providers of CYP Personalised Healthcare Commissioning.
We have over a decade of experience in young people’s continuing care, such as complex care, S117, mental health, learning disability and autism.
Our operational team includes registered children’s nurses, health visitors, school nurses and other professionals with expertise in CYP universal and specialist services.
We offer training and competency services to ICS teams in the specialist field of CYP services, including best practice for transition to adulthood and safeguarding.
We can also support with outstanding CYP 3- and 12-month reviews, appeals, independent reviews and complex case reviews, acting as an independent party to support investigations.
We can:
- Tailor our service to suit your requirements, be it complete end-to-end or modular support
- Deploy our specialist CYP workforce to meet service needs
- Develop protocol, policy and standards in conjunction with system partners to meet needs of local population
- Support the Independent Review Process – for cases in appeal, dispute resolution and service user complaints
- Provide Independent Clinical Health Assessments, for both individual and multiple cases
- Provide caseload management – including complex physical and mental health needs of CYP
- Provide ongoing support and clinical supervision across staff groups.
