Western Avenue Medical Centre in Chester partnered with our Medicines Management and Optimisation team to conduct face-to-face medication reviews for high-risk patients. Over 25 sessions, our clinical pharmacist performed 80 reviews, resulting in 241 quality interventions and 33 cost-saving interventions, saving £802.45. These reviews improved patient safety, optimised prescribing, and led to the development of medication audits to further enhance clinical practices.
