Through the Me and My Medicines campaign, we empowered patients to raise concerns and use their medicines more effectively, improving overall health outcomes. Partnering with NHS Greater Preston and NHS Chorley and South Ribble CCGs, we piloted the programme in one GP practice and later expanded it to nine. Over 257 patients with diverse comorbidities participated, leading to significant quality interventions, medication adjustments, and positive feedback from patients and healthcare professionals alike.
