In collaboration with Warrington GP practices, our Medicines Management and Optimisation team conducted a comprehensive review of Direct Oral Anti-Coagulants prescribing. The aim was to improve patient safety, ensure correct dosing, and prevent unnecessary prolonged treatments. The review resulted in better monitoring and documentation of patients’ renal function and weight, as well as a £9,000 cost saving. The project identified key areas for improvement, such as creating clinical system protocols and implementing alerts for timely reviews.
