Our intensive care home medication review in Wirral resulted in significant improvements in pharmaceutical care. By optimising medication use, reducing waste, and enhancing communication between care home staff, GPs, and pharmacists, we achieved direct cost savings of £10,728 annually and reduced medication errors.
