Our pharmacist-led Polypharmacy Review Service, commissioned by Birmingham and Solihull CCG, improved patient safety and reduced healthcare costs. By de-prescribing unnecessary or harmful medications and addressing patient concerns, the service has enhanced quality of life for people with long-term conditions and reduced the risk of hospital admissions. With over 4,225 medications stopped and 307 potential hospital admissions avoided, the service achieved a cost improvement of just over £1 million.
