The harmonisation project led by our Medicines Optimisation team successfully established governance and streamlined formulary reviews. By creating a collaborative working group with pharmacists from primary and secondary care, the project improved consistency in medication prescribing across Cheshire and Merseyside. This initiative not only enhanced patient care but also increased operational efficiency within the NHS Cheshire and Merseyside Integrated Care Board.
